BetterHelp Enrollment Form

Welcome to the BetterHelp Enrollment Form. Please read the following guidelines before completing your submission: 

1. Eligibility 

• This program is available only to active KFC restaurant employees or their dependent(s) (up to 2 dependents under the age of 18 per employee). You will be asked to confirm your eligibility. 

2. Required Information

• Your TeamKFC ID
• Your personal email address, which will be used to send you a link to sign up for BetterHelp
• If you are a dependent, you will need to provide the TeamKFC ID of the employee you are associated with.

3. Privacy and Confidentiality 

• Your TeamKFC ID is used only to confirm eligibility and will not be shared with BetterHelp or KFC.
• Your personal email will be used only to send you sign-up information and will not be used for any other purpose.
• All services received through BetterHelp are completely confidential and not accessible by KFC. 

4.  Ineligible Applicants 

• If you are not an active KFC restaurant employee or a dependent of one, you will not be eligible for this program.
